William Whitman, a patient at the Kidney Institute, successfully reversed his stage 4 kidney failure progression under Dr. Bismah Irfan's care. Whitman reported his GFR nearly doubled from 15 to 29, while his creatinine levels dropped from 3.9 to 2.35. By utilizing functional medicine, he avoided scheduled dialysis and restored his energy levels at age 78.

The interview
What specific improvements have you seen in your lab numbers?
My GFR is what I looked at was down to 15 and now it's up to like 29. So it's almost doubled. My creatinine was up like 3.9 and it's down to 2.35 I think. Slowly every time I get a blood test, I get better.
What were your struggles before finding the Kidney Institute?
I was in stage 4 kidney failure and it was getting worse. My numbers were going down or up, as it may be, and was faced with dialysis as an option. Basically, the physicians group were preparing me for dialysis, sending me to classes for me and my wife, and didn't see any hope for the future for me to stay off of dialysis.
How did kidney failure affect your daily life?
The biggest effect on my daily life was fatigue, constant fatigue. My kidneys weren't functioning well and I was tired all the time. I'm 78 years old. I own my own business. I'm still working. I was falling asleep during the day and I'm too tired to go out and all this stuff I want to do.
How did you find Dr. Irfan and the Kidney Institute?
My daughter is an ER, emergency room physician; she works for a Cleveland Clinic. And she knows Dr. Irfan. She said to me, 'You need to call this lady.' As a result of that, my wife and I contacted Dr. Irfan.
What was your first impression of Dr. Irfan's approach?
She was very direct, and she asked a lot of questions, and she seemed right out of the box to say, 'I'm gonna try and help you if I can. Let's look at what your options are.' She believed there was a good chance we could improve my GFR and creatinine and change levels of uric acid.
How did this compare to your previous medical experiences?
What I experienced beforehand was, 'well, these are your numbers and you're in decline and this probably isn't going to get any better.' They told me my age is against me and I have one kidney. They basically were taking me down the road to dialysis.
Were you skeptical about the online care model?
I was skeptical. I was argumentative and I was, 'are you sure this is gonna work? How in the heck is somebody in Texas gonna take care of me?' But I don't think there's any difference than sitting in a doctor's office. When I call, Dr. Irfan sees me every time. She can order any blood test, scan, or x-ray I need here.
Frequently asked
Can online care effectively treat kidney disease from another state?
William Whitman was initially skeptical about how a doctor in Texas could care for him in Indiana, but found that Dr. Irfan could order all necessary blood tests, scans, and prescriptions locally. He noted that the online visits were more reliable and accessible than his local family physician.
Is it possible to improve kidney function with only one kidney at age 78?
Despite being told by conventional physicians that his age and having only one kidney meant his condition would only get worse, William Whitman saw his GFR nearly double. He reported feeling 'back to being me' with significantly higher energy levels for his business.
What is the communication like at the Kidney Institute?
Whitman highlighted that he has direct access to Dr. Irfan during scheduled visits and can reach her assistant, John, via telephone for immediate answers. He found this level of service superior to traditional offices where doctors are often too busy to see patients personally.
